Medical Social Worker (LMSW)
Hospital-Based | Full-Time | Monday–Friday
We are seeking a compassionate and motivated Medical Social Worker to join a hospital-based care team. This role is ideal for a licensed social worker who thrives in a fast-paced inpatient/unit setting and is passionate about supporting patients through effective discharge planning and care coordination.
Position Overview
The Medical Social Worker will work directly within a hospital unit, managing a daily caseload of approximately 25 patients . In this role, you will collaborate closely with a Nurse Case Manager (NCM) and the broader multidisciplinary care team to ensure safe, timely, and appropriate discharges. This is a hands-on, patient-facing position within a hospital environment — not an office or outpatient setting .
We are open to newer LMSWs as long as they are comfortable working in a hospital/unit setting and managing a high-volume caseload. Candidates with prior hospital experience are strongly preferred.
