Schedule & Compensation:

  • Full-Time (40 hours per week)
  • Hours: 8:00 AM - 4:30 PM (flexible)
  • Salary: $55,000/annually

Job Description

  • Develops and maintains professional relationships with staff, physicians, residents, families, community resources, and third-party payers.
  • Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process.
  • Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly.
  • Assess residents various health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.

Qualifications

  • Experience in Long-Term Care or medical setting preferred
  • Comfort and experience working with dementia populations a plus
  • Flexibility to assist in our skilled, assisted, and residential living areas
  • A four-year undergraduate degree in social work, psychology, or a related field.
